The Oregon Trail Community Foundation's Book of Memories is a wonderful way to remember someone who has been important in your life, or the life of a friend or business associate. All entries are welcome, and will promptly be recorded in this historic book as a memorial to the deceased. The Book of Memories is currently on display inside the west entrance of Platte Valley Bank (Main Bank).
Please print and return the donation form if you would like to add someone to the Book of Memories. A minimum donation of $20.00 is recommended.

Introduces Prepaid Memorials
HOW DOES IT WORK?
On the occasion that someone you know passes on, simply fill out a “Book of Memories” card provided by OTCF (see enclosed sample) and mail it to our office, preferably with a copy of the obituary. You may select the amount you wish to donate (a minimum of $20.00 is recommended). The card has an area in which you can designate who is to receive an acknowledgement of the donation. A card is then sent to the family, advising them you or your company has made a donation in honor of their loved one and he/she will be inscribed in the Book of Memory. The amount is not disclosed.
This simple procedure continues until your prepaid amount is almost depleted. OTCF will then send you an itemized list of memorials you have donated to and the balance of your account. At that point, you may send another donation.
