The Oregon Trail Community Foundation's Book of Memories is a wonderful way to remember someone who has been important in your life, or the life of a friend or business associate. All entries are welcome, and will promptly be recorded in this historic book as a memorial to the deceased. The Book of Memories is currently on display in the lobby of the Star-Herald.

Once you fill out and send the 'Book of Memories form' along with your tax-deductible gift, the Foundation will send the family of the deceased a letter informing them of your memorial donation (the amount will not be mentioned) and that their loved one's name will be entered into the Book of Memories. It will also tell them where the Book is displayed.

Please print and return the donation form if you would like to add someone to the Book of Memories. A minimum donation of $20.00 is recommended.

Introduces Prepaid Memorials

HOW DOES IT WORK?
Your firm, or you personally, send OTCF a lump sum check (i.e. $200 to $400.00). You or your company’s account is credited with this amount, and you or your company will receive the required IRS tax letter (sample enclosed) acknowledging your donation.

On the occasion that someone you know passes on, simply fill out a “Book of Memories” card provided by OTCF (see enclosed sample) and mail it to our office, preferably with a copy of the obituary. You may select the amount you wish to donate (a minimum of $20.00 is recommended). The card has an area in which you can designate who is to receive an acknowledgement of the donation. A card is then sent to the family, advising them you or your company has made a donation in honor of their loved one and he/she will be inscribed in the Book of Memory. The amount is not disclosed.

This simple procedure continues until your prepaid amount is almost depleted. OTCF will then send you an itemized list of memorials you have donated to and the balance of your account. At that point, you may send another donation.
